One-question rating survey email template
One question is easy to answer, so this survey asks only one and makes every answer a tap. Each number from 0 to 10 is its own button, a short note says why you ask, and a last button is there for anyone who would rather write.

What's in this template
- Bright band: 0 to 10 headline and the question, with what each end means
- Keypad of 11 numbered buttons, 3 to a row
- Short note on why you ask
- Dark band with a button: Write a note instead
When to use it
Use it for a recommendation score a few weeks after signup, after a support case, or once a quarter for existing customers. Software teams, gyms, schools and membership groups can all use it. Link each number to a page that records the score and then asks for a sentence.
Make it yours
- Rewrite the question to name your product, and say what zero and ten mean.
- Link each numbered button to its own address so the tap records the score.
- Change the bright band to your brand color; the buttons take their outline from it.
