One-question customer survey email template
A survey gets answered when it asks for one tap. This one puts a single question up front with ready-made answers as buttons, then tells the reader what happens to the replies, which is the part most surveys leave out.

What's in this template
- Teal strip: one question, two minutes, anonymous
- Headline question with a short note to the reader
- 4 full-width buttons, the last one for something else
- Coral band on how the answers get used
- Sign-off with a photo, a name and a role, then a thank-you and a P.S.
When to use it
A few months after someone signs up or buys, once they have had time to form an opinion. It also works after a support chat or an event. Stick to the one question and ask for more only after someone replies.
Make it yours
- Rewrite the question and answers, and adjust "three months in" and "anonymous" to what is true.
- Name one real change from the last round in the note, so readers see their answers lead somewhere.
- Sign it with the person who will actually read the replies.
